What Is a LLETZ Procedure?

A LLETZ procedure (Large Loop Excision of the Transformation Zone) is a minor treatment used to remove abnormal cells from the surface of the cervix using a thin heated wire loop. The aim of the procedure is to remove the abnormal area while preserving as much healthy cervical tissue as possible. It is one of the most commonly performed and effective treatments for moderate and severe cervical cell abnormalities identified following cervical screening and colposcopy.
The tissue removed during the procedure is sent to a specialist laboratory for detailed examination to confirm the diagnosis and assess whether the abnormal cells have been completely removed where appropriate. By treating abnormal cervical cells before they have the opportunity to progress, a LLETZ procedure plays an important role in preventing cervical cancer and protecting your long-term cervical health.

Why Might I Need a LLETZ Procedure?

A LLETZ procedure is usually recommended when cervical screening, HPV testing, and colposcopy identify moderate or severe cervical cell abnormalities that are unlikely to resolve without treatment. Removing these abnormal cells at an early stage helps prevent them from progressing to cervical cancer and is an important part of protecting your long-term cervical health. The decision to recommend treatment is based on the results of your individual assessment and the severity of the cervical cell changes identified.

Your consultant will explain your cervical screening, HPV, biopsy, and colposcopy results in clear, straightforward language and discuss why a LLETZ procedure has been recommended. We will also explain the expected benefits, any potential risks, and the alternative management options where appropriate, ensuring you feel fully informed and confident before making a decision about your treatment.

What Does the Procedure Involve?

Your appointment will begin with a detailed discussion about your medical history, cervical screening results, HPV status, biopsy findings, and any questions or concerns you may have. Your consultant will explain the LLETZ procedure step by step, discuss what you can expect during and after treatment, and ensure you fully understand the procedure before asking for your consent to proceed. Our aim is to help you feel informed, comfortable, and reassured throughout your appointment.

The procedure is usually performed under local anaesthetic during a colposcopy appointment and typically takes only a few minutes. Most women experience some pressure or mild discomfort rather than significant pain. Once the procedure has been completed, your consultant will explain your aftercare, discuss when your laboratory results are expected, and arrange any recommended follow-up to monitor your recovery and long-term cervical health.

Recovery After a LLETZ Procedure

Most women return home shortly after treatment and are able to resume their normal daily activities within a day or two. Mild cramping and light vaginal bleeding or discharge are common during the healing process and usually settle over the following few weeks. Although recovery is generally straightforward, allowing the cervix time to heal properly is an important part of achieving the best possible outcome and reducing the risk of complications.

Your consultant will provide detailed aftercare advice tailored to your individual circumstances, including guidance on exercise, sexual intercourse, swimming, bathing, and tampon use while the cervix heals. We will also explain any symptoms that should prompt medical review and arrange any recommended follow-up, including cervical screening or colposcopy where appropriate, to ensure your recovery is progressing well and your long-term cervical health remains protected.

LLETZ and Future Fertility

Many women are understandably concerned about how a LLETZ procedure may affect their ability to have children in the future. For the vast majority of women, a LLETZ procedure does not prevent pregnancy, and most go on to conceive naturally and have healthy pregnancies. Understanding the potential effects of treatment before your procedure can help you make informed decisions and provide reassurance about your future reproductive health.

Although a small increase in the risk of cervical weakness or preterm birth has been reported following larger cervical excisions, this risk varies depending on the amount of cervical tissue removed and your individual circumstances. Your consultant will discuss any potential implications for future pregnancies, answer your questions, and ensure that treatment is planned as carefully as possible while effectively managing the abnormal cervical cells.

Frequently Asked Questions

Is a LLETZ procedure performed under general anaesthetic?

A LLETZ procedure is usually performed under local anaesthetic during a colposcopy appointment, so a general anaesthetic is not normally required. The treatment is performed on an outpatient basis, allowing most women to return home shortly after the procedure.

How long will my appointment take?

Although the LLETZ procedure itself usually takes only a few minutes, your appointment will typically last longer to allow time for consultation, preparation, treatment, and aftercare advice. Your consultant will also answer any questions you may have before and after the procedure.

Will the tissue be tested?

Yes. The cervical tissue removed during the procedure is routinely sent to a specialist laboratory for detailed examination. This confirms the diagnosis, helps ensure the abnormal cells have been adequately treated where appropriate, and guides any follow-up that may be required.

When will I receive my results?

Your laboratory results are usually available within a few weeks. Once they have been received, your consultant will explain the findings clearly, discuss whether any further treatment or follow-up is required, and answer any questions you may have.

Consultant examining with colposcope equipment
Are there any activities I should avoid after treatment?

Yes. While your cervix is healing, you will usually be advised to avoid sexual intercourse, tampon use, swimming, and strenuous exercise for a period recommended by your consultant. Following your aftercare instructions helps reduce the risk of bleeding and infection and supports normal healing.

Will I need follow-up after a LLETZ procedure?

Yes. Follow-up is an important part of your care. Depending on your individual circumstances and laboratory results, your consultant may recommend further cervical screening, HPV testing, or colposcopy to confirm that treatment has been successful and to monitor your long-term cervical health.

When can I return to work?

Many women feel well enough to return to work and their normal daily activities within one or two days, although this varies depending on the individual and the nature of their work. Your consultant will advise you on when it is appropriate to resume your usual activities.

Do I need a GP referral?

No. Many women choose to self-refer for private assessment and treatment. Referrals from GPs, cervical screening providers, and other healthcare professionals are also welcome.

Doctor writing on a referral form
Can I have a second opinion before deciding on treatment?

Yes. If you have been advised to undergo a LLETZ procedure and would like reassurance before proceeding, our experienced consultant gynaecologists can review your cervical screening results, colposcopy findings, biopsy results, and treatment recommendations before discussing the options available to you.

How quickly can I be seen?

We provide fast access to consultations, colposcopy, LLETZ procedures, and follow-up care, helping women receive prompt assessment, timely treatment, and expert support without unnecessary delays.
